The README had drifted from the actual code. This PR brings it back in line with what's on `staging`.

## Before / After
**1. Default model provider**
- Before: "Supports xAI (default), OpenAI, Fireworks, and other model providers" and a Model Providers section claiming the default is xAI `grok-2-vision-1212` / `grok-3-mini-beta`.

- `AGENTS.md` repeats the "DB migrations run automatically during `pnpm build`" claim, which is also inaccurate.
